ACT Criteria

ACT serves adults with serious and persistent mental illness.  Individuals who benefit most from ACT intensive community treatment are generally people who are diagnosed with schizophrenia, other psychotic disorders and bipolar disorder.  In addition, many consumers have co-occurring substance use disorders.  ACT consumers must have all of the following:

  • Patterns of frequent hospital admissions and/or incarcerations
  • Frequent use of emergency services
  • Severe problems completing activities of daily living
  • Recurrent homelessness
  • Recurring involvement with the criminal justice system
  • Non-adherence to medications as prescribed
  • Have not improved with traditional case management services
